Section 88.4 - Code of Iowa (2001) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling objects: (a)Magnesium Melt Room - Employees using below the hook lifting devices were exposed to hazards of falling objects which could result in burns or crushing injuries. The below the hook lifting devices (Hooks/Tongs) employees were using appeared to be bent or sprung. The employer did not provide below the hook lifting devices designed and marked as specified in ASME B 30.20 1993 Section 20.-1.2. This violation was noted on or about 06-20-01. One feasible and acceptable abatement method, among others, to correct this hazard is to comply with ASME B 30.20 1993. (b)Dry Sand Area - Employees using below the hook lifting devices were exposed to hazards of falling objects which could result in burns or crushing injuries. The below the hook lifting devices (Lifter on the Transfer Buggies) employees were using, had steel plates welded on the lifting frame for reinforcement purposes. This lifting frame had a hole completely through the weld of a upper corner brace. The employer did not provide below the hook lifting devices as specified in ASME B 30.20 1993 Section 20. 1-2 and Section 20 -1.2.2. This violation was noted on or about 06-20-01. One feasible and acceptable abatement method, among others, to correct this hazard is to comply with ASME B 30.20 1993.

Section 88.4 - Code of Iowa ( 2001) 88.4 : The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to serious burns: (a)Magnesium Melt Room & Dry Sand Area - Employees were exposed to serious hazards of burns from Hot Molten Metal during its transfer by transfer buggies. The employer did not provide warning devices on transfer buggies as specified in ANSI Standard Z 241.2 Section 5.2.5. This violation was noted on or about 06-20-01. One feasible and acceptable abatement method, among others to correct this hazard is to comply with ANSI Standard Z 241.2 1989.
