Section 88.4 - Code of Iowa (2001) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing/amputation hazards: (a)Back Room - The safety interlocks for the Fox Hydraulic Baler's loading chamber closure and the bale chamber door were disabled or bypassed. This allowed the ram to operate in a compression stroke while the chambers were unguarded. On or about 4/16/01, an employee received a severe injury while operating the baler. This condition was noted on 5/15/01. Among other methods, one feasible and acceptable abatement method to correct the hazard is to implement an inspection program to ensure that the safety interlocks are functional, as outlined in clause 7 of ANSI Z245.5 1997.
