Section 88.4 - Code of Iowa (2001)
88.4:  The employer did not furnish employment and a place of employment
which was free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to confined space entry:
(a)Dormitory Attics and Tunnels - Employees are entering these confined
spaces
when installing the fire protection system.  The company has not
implemented
a confined space entry program that ensures the employees follow safe entry
procedures.  This could result in the employees being injured when caught
in
unguarded belt/pulleys, exposed electrical parts or entangled in existing
utility
piping.  Also employees could be exposed to an atmospheric hazard should
the
asbestos insulation be disturbed.  Among other methods, one feasible and
acceptable abatement method to correct the hazard is to implement a
Confined
Space Entry Program in accordance with ANSI 117.1.ance
