crushed by the industrial lift truck.
a) Shop area - Employees were operating a Clark industrial forklift truck
that was not equipped
or retro-fitted with seatbelts.  This was exposing the operators to
struck-by/crushed-by hazards,
should the forklift be accidentally tipped over, that could result in
fatal injuries when their upper
body parts were struck/crushed by the forklift truck's overhead protection
device as they were
thrown out of the operator seat. On or about 03/12/02.
Other feasible and acceptable abatement methods to correct this hazard are:
Install seatbelts on the industrial trucks and require employees to wear
the seatbelts.
