truck to a height of
approximately twelve feet to perform work perform work on the steel beams
of the parking deck
under construction.  No fall protection was provided to the employee.
Violation observed on or about 8/23/00
Abatement Notes: Feasible and acceptable abatement methods to abate this
hazard include but
are not limited to:
1) Use a safety platform with guard rails secured to the lifting carriage
or forks
2) Use of a scissor lift with guard rails on all four sides
3) Follow the requirements of American National Standards B56.1-1993,
Safety Standard for
Low Lift and High Lift Trucks, Section 4.17, Elevating personnel.
