employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards from the special purpose personnel elevator:
The special purpose personnel elevator located in the grain elevator did
not have electrical
interlock switches or means of locking the doors shut while the elevator
was in motion as
required by the American Society of Mechanical Engineers in Safety Code
