Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of explosion/implosion: (a) Location - Production area: On or about 6/20/02, employees were exposed to the hazard of explosion/implosion due to ethylene glycol kept at elevated temperature (150F) and pressure (up to 15 p.s.i.) in metal tanks designed and manufactured for liquid oil storage, which were altered by the welding, cutting and installation of valves and heaters. Feasible and acceptable abatement methods to correct this hazard include, but are not limited to, the following: 1. Evaluate tanks for their present use including, but not limited to, the risk of brittle fracture, hydrostatic testing, identification of materials used in alterations and repairs, impact testing of welds, and examination of new penetrations, by an engineer experienced in storage tank design, as mandated by American Petroleum Institute Standard 653 (2001). 2. Design and construct tanks that are certified by a professional engineer for the process of purifying an ethylene glycol mixture.
