were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazard of falling:
(a) Location - Liberty Tree Mall, Danvers, Ma:
On or about 06/25/02, employees were exposed to serious fall hazards of up
to 32 feet
while climbing out of a scissors lift to access the upper floors.
Among other methods, feasible and acceptable, to correct this hazard,
include but are not
limited to: a) Conduct training on the safe use of the scissors lift as
per manufacturer's
operating instructions: b) Utilize operating instructions as specified by
ANSI/SIA A92.6-
1990 for self propelled elevating work platforms. c) Enforce employees to
use the ladder
access system set-up within the structure.
