Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazard of falling: (a) Location - Liberty Tree Mall, Danvers, Ma: On or about 06/25/02, employees were exposed to serious fall hazards of up to 32 feet while climbing out of a scissors lift to access the upper floors. Among other methods, feasible and acceptable, to correct this hazard, include but are not limited to: a) Conduct training on the safe use of the scissors lift as per manufacturer's operating instructions: b) Utilize operating instructions as specified by ANSI/SIA A92.6- 1990 for self propelled elevating work platforms. c) Enforce employees to use the ladder access system set-up within the structure.
