Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
are exposed to amputation hazards due to unexpected start up of the
Wor-Tex JC-10:
(a) Location - Production Area:
On or about 09/04/03, employees are exposed to amputation hazards due to an
inoperative magnetic trip switch on the Wor-Tex JC-10.
Feasible abatement methods include but are not limited to the following:
1. Replace the magnetic trip switch with a new switch from the
manufacturer, or replace
with a switch that is of equal integrity to perform the function of
properly safeguarding
the rotating parts.
2. Implement procedures in the preventative maintenance program for the
Wor-Tex JC-
10 to ensure the magnetic trip switch does not malfunction due to the
build
up of plastic
granules.on
