North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North
Carolina:  The employer did not furnish each of his employees conditions
of employment and
a place of employment free from recognized hazards that were causing or
likely to cause death
or serious physical harm to employees in that employees were exposed to
contact with electricity
and water from bathroom facilities
a)Administrative office bathroom- the electric receptacle did not have
a Ground Fault Circuit Interrupter (GFCI) installed in the
bathroom as required by the National Electric Code, Article 210-8
(b) (1).
b)Fabrications shop bathroom- the electric receptacle did not have a
Ground Fault Circuit Interrupter installed in the bathroom as
required by the National Electric Code, Article 210-8 (b) (1).
Among other methods, one feasible and acceptable method to correct this
hazard
would be to
have Ground Fault Circuit Interrupters installed in both bathrooms.

falling hazards due
to no inspection documentation showing equipment serviceability and no
operating procedures
being on hand. Ref:ANSI B30-1981 Sec.16-1.1.4 and Sec.16-2.1
a)Fabrications shop- The chain hoist had no records of inspections
being accomplished.
b)Fabrications shop- The chain hoist had no labels displaying
operating procedures.
Among other methods that are feasible and acceptable to correct these
hazards would be to
inspect, maintain and label the hoist in accordance with ANSI B30-1981 Sec
16-1.1.4 and Sec.
16-2.1.4
