North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to rotating unguarded shafts by having to manually move heavy material on overloaded conveyor equipment: a)building #1/press #1 handling system-On or about 11/13/00, the following conditions occurred: The belt conveyor of the holding table could not handle the specified loads resulting in employees having to manually move material on the holding table. Approximately 3,169 pounds of Die #610789 and 3,830 pounds of Die #230468 aluminum metal was on the belt conveyor of the holding table. (Ref: Sec.5.1, ANSI/ASME B20.1-1984) one feasible and acceptable method, among others, to correct the hazard is that where ratings are not available, good industry practice shall be used.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that the starting/stopping device was not kept free from obstructions: a)building #1/press #1 handling system-On or about 11/13/00, the following conditions occurred: A 25' steel tape was lodged in the foot pedal of the holding table operating controls located at the Cut-off saw used to operate the belt drive conveyors, keeping the conveyor in continuous operation. (Ref: Sec. 5.12(b) ANSI/ASME B20.1- 1984) one feasible and acceptable method, among others, to correct the hazard is for an employee to operate the foot pedal to control the belt conveyors of the handling system.

(Ref: Sec. 5.12(i) ANSI/ASME B20.1-1984) one feasible and acceptable method, among others, to correct the hazard is for an employee not to wear loose clothing while working near conveyors.
