improperly constructed job made ladder (Ref: ANSI A14.4-1979,
paragraphs 4.5.2 and 4.5.5):

Construction was observed installing sheathing on the
outside of the second level of the building, standing on
a at approximately 11'9" above the ground, on a job made
ladder that did not have filler blocks inserted between
the ladder cleats and had the cleats spaced at
approximately 18" instead of the required 12" +/- 1/2
inch.
