North Carolina:  The employer did not furnish to each of his employees
conditions of
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees
were exposed to:
a)being crushed or caught under the powered industrial truck in the event
of an
accident.  Employees in the manufacturing area were not using the seat
belts
while operating the caterpillar 50, model #2EC25, Serial #A2EC262549
powered industrial truck.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to enforce the seat belt requirement for employees
who
operate the powered industrial trucks in accordance with the operators
manual
and wear the seat belts in accordance with the operators manuals.
