Carolina:  The employer did not furnish to each of his employees
conditions of employment and
a place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees in that employees were
exposed to:
a)warehouse area - being crushed by or caught between the forklift and the
ground
in the event the vehicle was up-set or turned over.  The operator was not
wearing
the installed seat belts while operating the 3000 lb, Toyota forklift, s/n
15458,
model 42-3FGC15.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to require and enforce the operators to wear seat belts while
operating the
forklifts.
