North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North
Carolina:  The employer did not furnish to each of his employees
conditions of employment and
a place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees in that employees were
exposed to:
a)being struck by the structure should it fail - throughout the facility
such as the bed
of a trailer located in shipping - monorail crane was not load tested
prior to initial
use.
NOTE:Among other methods, one feasible and acceptable method to correct
this
hazard is to have all cranes load tested.

throughout the
facility such as the Roll Mill Area - operational tests were not performed
on all
below the hook lifting devices prior to initial use.
NOTE:Among other methods, one feasible and acceptable method to
correct this hazard is to have all below the hook lifting devices
tested prior to initial use.
