Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed the hazard of falling:
(a) Location - 231 Bainbridge Street, Malden, MA:
On or about 10/02/02, an employee was exposed to a fall hazard of up to 25
feet, while
working in a vehicle-mounted Holan Aerial Lift, due to mechanical failure
of the
hydraulic system.
Among other methods, feasible and acceptable, to correct this hazard,
include but are not
limited to: perform and record inspections and tests on the
vehicle-mounted aerial lift at
least annually in accordance with the American National Standards
Institute (ANSI) /
Scaffold Industry Association (SIA) A92.2-1990, American National Standard
for
Vehicle-Mounted Elevating and Rotating Aerial Devices.
