Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to: possible fall to hoistway (elevator shaft) bottom from
first floor landing.
On or about April 6, 2001, on first floor, the freight elevator gate was
found to have the
electrical interlock taped closed so that the elevator could be operated
with the gate up.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD  TO CORRECT THIS HAZARD IS:
To maintain the electrical/mechanical interlocks in good repair. Meeting
the requirements of
ANSI A17.1-1971
Section 111 - Hoistway-door locking devices, car door or gate electric
contacts, hoistway access
switches, and elevator parking devices;
Section 1002 - Maintenance, Rule 1002.4   Making Safety Devices
Inoperative.  No person shall
at any time make any required safety device or electrical protective
device inoperative, except
where necessary during tests, inspections, and maintenance.  Immediately
upon completion of
such tests, inspections, and maintenance, such devices shall be restored
to their normal operating
condition  in conformity with the applicable requirements (see Rule 210.7)
