Code of Virginia - 40.1-51.1(a): The employer, Packaged Ice, Inc./dba Reddy Ice, did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that the employer did not ensure that the automatic reversing mechanism on sliding bay doors to the ice freezer and bagging areas were maintained in operable condition to ensure that employees were not exposed to the hazard of being caught between the door and the door frame, in the following instances: A. Located in the #6 Cooler Plant Loading Dock area, where employees operate forklifts, tow motors, and walk in and out of the work areas in Bagging Room 5, the automatic reverse mechanism on the #5 Sliding Bay Door was inoperable when tested. B. Located in the #6 Cooler Plant Loading Dock area, where employees operate forklifts, tow motors, and walk in and out of the work area in Cooler #6, the automatic reverse mechanism on the #6 Cooler Sliding Bay Door was inoperable when tested. One feasible and acceptable method among others to correct this hazard is to maintain the equipment in the same condition in which it was built by the manufacturer and to adhere to Underwriters Laboratories Standard #325 - Door, Drapery, Gate, Louver and Window Operators and Systems - Subpart 51.6 for Industrial and Commercial Vehicular Doors and Door Operators which requires ...periodic checking of the control mechanisms. and the provision of ...clear instructions for the installation of reversing edge mechanisms.n
