40.1-51.1(a) of the Code of Virginia: The employer, Des Champs
Laboratories, did not furnish a place of
employment which is free from recognized hazards that are causing or
likely to cause death or serious physical harm to
employees in that employees are operating a Promecam Press Brake, #RG-103,
which exposes them to amputation by
not having a point of operation guard in place.
Operators are flattening stock,  which is 1 1/2 " to 2" in width,  with
the Promecam Press Brake,.   This is a one
person operation which requires the fingers of the operator to be at the
point of operation while holding the material.
One feasible and acceptable abatement method (among others) to correct
this hazard is to install a point of operation
guard such as that called for by ANSI B11.3-1982, American National
Standard Safety Code for Power Press Brakes.

employees in that employees operate electric hoists without the load
ratings on supporting structures having been
checked and posted.
In Areas 13E and 42C, the supporting beams and the chains used to secure
two Harrington 5 Ton Peerless electric
hoists have not been provided with load ratings.  This exposes employees
to contusions and fractures by not having the
load ratings on the supporting structures at least equal to that of the
hoists.  Both hoists are lifting materials which are
appx. 1 ton.    One feasible and acceptable abatement method (among
others) to correct this hazard would be to
provide the load ratings on the supporting structures such as that called
for by ANSI B30.16-2, American National
Standard Safety Code for Electric Hoists.c

Amada NC - 9EX Press Brake, which is located in area D30, which exposed
them to:
(a)  Amputations by not providing adequate safeguarding to the back of the
press
brake to prevent the operator and
other employees from injury from the point of operation.  One feasible and
acceptable abatement method (among
others) to correct this hazard would be to provide an awareness barrier on
the back of the press brake to prevent
employees from coming too close to the machine such as that called for by
ANSI B11.3-1982, American National
Standard Safety Code for Power Press Brakes.
On September 30, 1999, the Des Champs Laboratories facility located in
Natural Bridge Station, VA., was issued a
serious citation for not providing adequate safeguarding to the back of a
Accurshear hydraulic shear.
