Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees:
As referenced in ANSI B11.5-1988 Safety requirements for construction,
care and use of
Ironworkers. Section 4.3.3 Single-Stroke, Section 4.3.4 Continuous Stroke,
Section 4.3.6 Hand
lever Control Design, Section 4.4.3 Motor Starter, and Section 6.4
Inspections and Maintenance.
Records
a) Employees performing work at Punching Station of Penninghouse
Ironworker Machine were
exposed to fingers and/or hands being caught in point of operation.
Feasible Method of Abatement is:
1) Machines using a full revolution clutches should incorporate a
single-stroke
mechanism.
2) The activation of continuous stroke function should be designed so that
activation of function
could be supervised, and initiation requires prior action or decision by
operator.
3) Methods to inhibit unintended hand lever operation.
1. Locking pins or latches
2. On/Off switch
3. Clutch lock
4) Incorporate the type of drive motor starter that disconnects the drive
motor from the power
source in the event of control-voltage or power-source failure, and
requires operations of the
motor when power conditions are restored to normal.
5) Perform periodic and regular inspections of the Penninghouse Ironworker.
6) Maintain records of inspections and maintenance work performed.
A search of the standard has been made and presently there are no
applicable standards that is
applicable.

operation by;
1. Recessed or flush buttons
2. Ring around the start button
4) Perform periodic and regular inspections of the Penninghouse Ironworker.
6) Maintain records of inspections and maintenance work performed.
7) Install a guard over drop-off side of Angle Iron Cutting Station.
