Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to crushing injuries from steer loaders in operation without operator knowledge of where floor material sorters are working. a) Material Recovery Facility - Bay # 3 - Bobcat Steer Loader - Model # 863 - Serial # 514426460 Loader Operator was in the process of continuing the transferring of cardboard recyclable to the bailer, when he did not verify the presence and location of the material floor sorter, prior to traveling in reverse, on or about 09/27/00. ABATEMENT VERIFICATION IS REQUIRED ONE FEASIBLE AND ACCEPTABLE METHOD OF ABATEMENT WOULD BE TO: 1) Follow the manufacturer's instructions in the training video entitled " Skid Steer Loader Operator Training Course", # 01-98-500/6722721, KCC-800-011, Safe Travel Section, which states, " Never start moving until you are sure that no one is in your path, look ahead, to the sides, and behind you, so that you know where everyone is". 2) Follow the Operator Course Handbook, page 13, which states, " Never start moving until you are sure that no one is in your path. Look ahead, to the sides, and behind you so that you know where everyone is". 3) Follow the Administrator's Guide for the Steer Loaders Operator Training Course, page 23, which states, " As the operator, it is your responsibility to know where everyone is and to look before you move the loader" 4) Follow the loader's safety manual, page 9, which states to " Keep others away from your operation" On page 20, it states, " Always look around before you back up, or swing an attachment. Be sure that everyone is in the clear".e
