ORS654.010:  The employer did not furnish a place of employment which was
safe and healthful
for employees, and did not do every other thing reasonably necessary to
protect the life, safety,
and health of the employees:
(a) An R.J.E. Telecom Inc employee was exposed to injury related to
potential fall hazards while
working at the Biehn Street Project in Klamath Falls, Oregon.  The
employee was working on
an extension ladder platform and was using an "E-Ladder Support" (sling)
while repairing an
existing splice on an aerial phone line strand.  The employee's safety
lanyard was connected to
the E-Ladder Support's screw clamp assembly tether (safety) chain.  The
employee had not
attached his safety lanyard to a separate attachment point, such as around
the strand.
THIS VIOLATION WAS COMPLIED WITH AT THE TIME OF INSPECTION.
