Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees employment and a place of employment
which were free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to the hazard of being struck by
or caught between
a Front End Loader and other Equipment:
(a) On or about 12/29/00 - Employer did not insure employees were trained
to operate
the front end loader in accordance with the manufacture instruction
manual.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to establish a training program that would instruct employees to
operate
equipment in accordance with the manufacturer's instruction manual, page
70 under
section "For Towing,Stopping Machine,Stopping Engine", also page 38 under
section
"For Procedures Before Starting", such as, if possible, park on level
ground, otherwise,
the wheels must be chocked, set the parking brake, and turn off the motor
when leaving
the controls, and move the gear selector to neutral.

walk around
inspections on the front end loader to check for leaks or broken or loose
parts.
Among other methods, one feasible and acceptable abatement method to
correct
this
hazard is to require employees to conduct an inspection on the equipment
prior to use to
identify any defects, in accordance with manufacturer instruction manual,
page 38 under
section "Procedures Before Starting".

hazard is to only allow employees who have the training and experience to
perform
maintenance, in accordance with manufacturer instruction manual, page 72
under section
"Rules When Servicing".

physical harm to
employees in that employees were exposed to the hazard of being caught in
conveyor equipment:
(a) On or about 12/29/00 - Employer did not have emergency stop buttons,
pull cords,
limit switches or similar emergency stop devices provided where employees
were
working at the edge of the shaker table pit conveyor system removing scrap
debris.
Among other methods, one feasible and acceptable method to correct this
hazard is to
install emergency stop buttons or similar devices at the pit area, as
prescribed by the
ANSI/ASMEB20.1-1990 5.11.2(b)(2)(c)(1), "Safety Standard for Conveyors and
Related
Equipment".
