(a)On or about 1 NOV 00, and at times prior thereto, a Toyota forklift
truck was
operated without the protection of a seat belt.  The truck was originally
manufactured with a seat belt and it had been removed.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to re-attach a seat belt to the forklift's seat.
