were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to potential fall hazards and/or crushing injuries
from incorrectly
guarded landings and elevator cars:
(a)General Feeds Division, Building #1, Receiving: The hand operated
elevator
was not installed in accordance with American National Standard Safety Code
For Elevators (ANSI A17.1-1960).  Deficiencies include, but may not be
limited to the rule noted below:
(1) The elevator car was not enclosed on the sides not used for entrance,
exposing employees riding the car to fall hazards. (Rule 601.1)
(b)Commodities Division, Building #18: The powered electric passenger
elevator
was not installed in accordance with American National Standard Safety Code
For Elevators Dumbwaiters and Escalators (ANSI A17.1-1960) and the Safety
Code For Existing Elevators and Escalators (ANSI A17.3-1986).  Deficiencies
include, but may not be limited to the rules noted below:
(1) The hoistway-doors did not have hoistway-door interlocks to prevent a
hoistway-door from being opened when the elevator is not at that landing
zone.  In the case that a door was opened while the car was not at the
landing,
employees on the upper landing would be exposed to an approximate 66 feet
high fall hazard, and employees on the lower level would be exposed to
crushing hazards. (ANSI A17.1, Rule 111.1a)
(2) The elevator car door was not so arranged to prevent opening more than
4
inches from inside the car when the car is outside the unlocking zone.

(c)Aqua Feeds Division, Building #5:  The powered electric passenger
elevator
was not installed in accordance with American National Standard Safety Code

include, but may not be limited to the rules noted below:

Among other methods, feasible and acceptable abatement methods to correct
these hazards
include:
- Compliance with applicable ANSI standards, including replacing the
hand-operated
car with one that is enclosed;
- Construction of an appropriate enclosure for the hand-operated elevator
car;
- Installation of appropriate interlocks on the electric elevators to
prevent hoistway-
doors and car-doors from opening unless the car is at the landing.
Note: Abatement certification and supporting documentation are required
for this item.
