North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)facility, box room -- the upper limit switch was inoperative on a 1 ton CM monorail overhead electric hoist. REFERENCE:ANSI B30.11-1980, paragraph 11-2.2.1(b)(4) Operational Testing. The hoist upper limit of travel device (the actuating mechanism of the hoist-limit device shall be located or adjusted so that it will trip the device in sufficient time to prevent contact of the load block or load with any part of the carrier or bridge. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to have the upper limit switch adjusted properly to prevent the load block or load from contacting the carrier.ng

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)facility -- initial load test were not conducted on CM Loadstar 1 ton monorail electric hoists throughout the facility before initial use of the hoists. REFERENCE:ANSI B30.11-1980, paragraph 11-2.2.2 Load Testing. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to have a load test of all overhead hoists used in the facility.

REFERENCE:ANSI B30.11-1980, paragraph 11-2.1.2 Frequent Inspection NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to conduct frequent inspections of the hoists as identified in the ANSI standards.
