a. On or about August 28, 2000, at the jobsite located at 400 Mercantile
Court in Wheeling,
Illinois, employees working on the prestress concrete slabs were exposed
to injuries from
falls due to the collapse of walking/working surfaces due to improper
engineering details
(BOCA 107.7), non compliance with permit (BOCA 111.3), and failure to
comply with
general professional architectural and engineering services (BOCA 114.1).
A feasible and useful means of abatement includes, but is not limited to
the following action:
1. Ensure that BOCA sections 107.7 (Engineering Details), 111.3
(Compliance with Permit),
and 114.1 (Professional Architectural and Engineering Services) are
followed.
