Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Employee(s) performing pneumatic leak tests of die cast assembly covers
were exposed to the
hazard of being struck by flying fragments from the covers if they were to
explode or burst
during the pressurization of the test fixture with nitrogen.
Among others, one method of abatement to eliminate this hazard would be to:
(1), use a type of regulator that, by design, would limit the nitrogen gas
flow into
the test fixture to the maximum needed for the test, i.e., 150 p.s.i.;
(2), enclose the test fixture with a device that would contain the flying
fragments
and isolate them from the operator or other employees;
(3), install a safety relief valve on the test fixture.
(4), follow the requirements found in ASME Boiler and Pressure Vessel Code,
Section VIII, Unfired Pressure Vessels, as referenced in OSHA Standard 29
CFR
1910.6.
Abatement documentation is required for this item in accordance with the
requirement of
29 CFR 1903.19(d).
