T.C.A. 50-3-105(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: fall hazards. In that at least one employee was documented as not using an appropriate cable climbing device and/or not staying tied off 100of the time while climbing a radio tower to approximately a 200' elevation. Among others, one acceptable method of abatement would be to use a cable climbing device attached to the provided cable and appropriately attached to a full body harness while ascending and descending the tower pegs.
