causing or likely to cause death or serious physical harm to

a)axle line -- Coffing 3 ton monorail hoist number 6B, was
damaged, in that it would loose load tension and allow
the load to fall up to 6 inches, and was not removed from
service.
NOTE:Among other methods, one feasible and acceptable method
to correct this hazard is to remove the hoist from
service until it has been repaired.e
