Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of a broken vision panel in the freight elevator hoistway door: a)First Floor, Elevator Landing: The vision panel of the freight elevator hoistway door was broken in order to allow an employee to reach an arm through the vision panel into the hoistway and open the elevator hoistway door without the elevator car being at the landing, on or about 01/19/01. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to repair the vision panel in the freight elevator hoistway door as described in ANSI / ASME A17.1-1993 : Safety Code for Elevators and Escalators, Part XII, Section 1200, Rule 1201.10 and Rule 110.7a.
