Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by falling hoisting equipment and load: a)Plant, 2nd Floor Milling Area: End stops were not installed on the open end beam, on or about 1/3/01 b)Plant, 2nd Floor Milling Area: Employees side-loaded the trolley, on or about 1/3/01 c)Plant, 2nd Floor Milling Area: Employees were permitted to stand under and not kept clear of suspended load, on or about 1/3/01 NOTE:Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1)Per the manufacturer's manual, install end stops on the open end beams by affixing a clamp or other barrier to the open end of the beam. 2)Per the manufacturer's manual, center trolley over the load when hoisting. 3)Per the manufacturer's manual, prohibit personnel from standing under the suspended load. This can be by establishing a zone where employees are prohibited from entering while the load is being suspended.
