Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards and when performing work inside the raised bucket of a loader. One feasible and acceptable method to correct this hazard is to use an approved man-lift system or scaffold system to perform work overhead. A) LOCATION: YARD AREA THE JOHN DEER LOADER WAS BEING USED TO ELEVATE EMPLOYEES IN THE BUCKET FOR MAINTENANCE PURPOSES AND THE LOADER WAS NOT EQUIPPED WITH VERTICAL ONLY OR VERTICAL AND HORIZONTAL CONTROLS USED TO LIFT A CARRIAGE OR FORKS THAT HAD A SAFETY PLATFORM FIRMLY SECURED.
