Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing or decapitation hazard.
a) Material Handling Hoist/Dumbwaiter:
All elevator hoistway-landing openings were not provided with a guard the
full height and width
of the opening.
Violation observed on or about 01/05/01.
Among other feasible and acceptable methods(s) of abatement:
1) To install a hoistway guard on each floor. Types may include horizontal
slide, single or
multisection; swing, single section; combination horizontal slide and
swing; or center-opening.
Each guard shall have electrical contacts, interlocked with the hoistway
system,
at each level;
preventing any door from opening, unless the hoist is stationary at a
level.
