Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. Receiving Department - On or about December 27, 2000, the elevator door on the second floor (dock area) did not have an operative interlocking device that would prevent access to elevator shaft when elevator car was in use and/or at the first floor level. Employees were thereby exposed to hazards associated with shaft opening. A feasible method of correcting the hazard is to conform to the specifications referenced in "ANSI/ASME A17.1 Section 204.4/1993.
