North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)fabrication department, employees operating a Mitsubishi forklift were not wearing seat belts while operating the forklift. NOTE:Among other methods, one feasible and acceptable method to abate this hazard is to enforce the use of seat belts during operation of the forklift.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)Fabrication, Cincinnati 90 ton hydraulic press brake serial #47552, light curtains were not set, utilized, and secured at the proper safety distance, which contributed to an employee being injured on October 31, 2000. b)Fabrication, Cincinnati and DiArco(TriArco) press brakes, employer was not conducting daily inspections of the machines to ensure proper operation, maintenance, and proper operation of safety devices/light curtains. c)Fabrication, employees were not adequately trained in the setting of light curtains, daily inspections, and proper usage of the curtains including the blanking option. NOTE:Among other methods, one feasible and acceptable method to abate this hazard is to develop a training program and utilize the safety distance formula as referenced through ANSI B11.3-1992- American National Standard for Machine Tools- Power Press Brakes Safety Requirements for Construction, Care, and Use. Also reference ANSI B11.19-1990- American National Standard for Machine Tools- Safeguarding When Referenced by the Other B11 Machine Tool Safety Standards- Performance Criteria for the Design, Construction, Care, and Operation.on,
