Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. Entire Facility - On or about November 26, 2001, the following elevator doors did not have an operative interlocking device that would prevent access to elevator shaft when elevator car was in use and/or at the basement level. Employees were thereby exposed to hazards associated with shaft openings. Basement, elevator doors - 1,3,5,7,9,11,13,15,17, and 19 First Floor, elevator doors - 1,3,5,7,9,11,13,15,17, and 19 Second Floor, elevator doors - 1,3,5,7,9,11,13,15,17 and 19 Third Floor, elevator doors - 1,3,5,7,9,11,13,15,17, and 19 A Feasible method of correcting the hazard is to conform to the specifications referenced in "ANSI/ASME," A17.1 Section 204.4/1993."
