Section 5(a)(1) of the Occupational safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a. On or about January 18, 2002, at the job site located at 811-23 South Western Avenue, Chicago, Illinois, 60612, an employee working from a Brambles Model S-65 boom aerial lift was exposed to a fall due to the lack of personal fall protection as required by the American National Standards Institute (ANSI A92.5-1992) Standard for Boom Supported Elevated Work Platforms.
