A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions associated with working with a manually propelled mobile gantry crane in accordance with the American National Standards Institute B30.5d - 1993, Qualifications for Operators: (a) 43rd Avenue pump station: Employees were using a Tri-Adjustable Gantry crane, model and serial number, unknown and had not received training in the hazards associated with the crane operations and unsafe handling of the crane.ng
