Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
For the period of time up to and including 04/03/02, at the job site
located at 1140 Fullwiller,
Abilene, TX 79606: Employees were exposed to struck-by hazards created
when the outrigger
floats of the crane being used on the job (a 200 Ton FMC Link Belt) were
placed on wire spool
ends that were cracked / broken (rather than timber mats / blocking), were
placed on unstable
blocking (rear float), were set on un-level surfaces that put the cylinder
to floats at angles other
than 90 degrees (all floats), which could allow the floats to slide or the
float collars to fail, all
of which could lead to the collapse / tip over of the crane.
Among others, some feasible and acceptable methods to correct these
hazards
would be to:
1) Construct proper timber mats / blocking (each mat at least three times
the size of it's
corresponding float).
2) Prepare the site by leveling the surface areas where the crane /
outrigger floats / timber mats
will be placed.
3) Train all operators to assure that the crane landing / footings areas,
floats / mats of any crane
being placed in operation are adequate and properly set up before making
the initial pick.
