Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
For the period of time up to and including 06/20/02, at the job-site of
Key Energy Services,
Inc., located 1/2 mile East of the intersection of East Erskine and the
Idalou Rd, Lubbock, TX
79401: Employees were exposed to the hazards of being struck-by a
collapsing work-over rig.
The conditions were created when the work-over rig guy-lines were
connected to unserviceable
and untested anchors (two of the anchors were bent and none of the anchors
were tagged to
indicate they had been tested within the recommended 2 year interval, as
per American
Petroleum Institute Recommended Practice 4G (RP 4G).
Among others, some feasible and acceptable methods to correct these
hazards
would be to:
1) Ensure that the physical condition of the guy-line anchors is verified
by performing a before
use inspection.
2) Verify, during the before use inspection, that the anchors have been
tagged to indicated that
they have been inspected / pull tested within two years.
3) Train work-over / pulling unit operators and supervisors that it is
their responsibility to ensure
that under no circumstance will these units rig up to any anchor that is
unserviceable, has not
been inspected, pull tested or tagged accordingly.
