Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
a)  The work site located at 2495 Grandview in Odessa,Texas; on or about
October 17, 2002,
employees were exposed to a water-injection/struck-by hazard associated
with the use of a high
pressure  8,000 psi control gun being used to water blast the interior of
a vessel at the site.
Subsequent analysis revealed that the control gun being used for the water
blasting operation had
been modified from the original manufactured length of 48 inches, to a
length of 34 inches, and
the  shoulder support and barrel handle were absent.
Among other methods, one reasonable means of abatement would be to assure
that control gun
assemblies are maintained according to the manufacturer's guidelines,and
that
employees are
thoroughly trained in the proper use and hazards of high pressure
equipment.
