40.1-51.1(a) of the Code of Virginia: The employer, Cave Hill Mechanical & Maintenance Services, Inc., did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were operating a 400 Ton Cincinnati Press Brake , located in the fabricating shop, which exposed them to amputations by not providing a cover for the foot treadle on the press brake to prevent accidental actuation by falling or moving objects, or by someone stepping on it. One feasible and acceptable abatement method, among others, to correct this hazard would be to provide a cover over the foot treadle such as that called for by ANSI B11.3-1982, American National Standard Safety Code for Power Press Brakes.
