employment and a place of employment which
were free from recognized hazards that were causing or were likely to
cause death or serious physical harm to
employees in that:
(a) a caught-between hazard existed when the manufacturer's
recommendations for safe operation of the five Hyster
Model 80 forklifts regarding the use of seat belts when the forklifts were
in use were not followed.
(b) the manufacturer had placed warning stickers in the cabs of the
forklifts that seat belts were required for safe
operation.
