Title 40.1-51.1.(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a) a fall from elevation hazard existed when the manufactures recommendations for safe operation of the Crown order picker #1233 regarding the use of the safety chain at the operator's compartment and the use of a safety belt and lanyard when the operator moves from the operator's compartment to the platform were not followed. When employee(s)/operator(s) on or off loaded packages from the elevated platform to and from a steel storage rack in the storage area, they were exposed to a fall hazard of approximately 18 feet to the concrete floor below. (b) the safety belt was stored on the order picker, the lanyard was wrapped around the overhead protection cage, and the safety chain could not be found. "ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THIS HAZARD IS TO FOLLOW THE MANUFACTURES' RECOMMENDATIONS AS WRITTEN IN THE OPERATING MANUAL AND AS PLACARDED ON THE OPERATOR'S WINDSHIELD.
