Title 40.1-51.1.(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a) At the south side of the office trailer, approximately 30 metal form pins, being used in curb and gutter forming operations and protruding 4 to 18 inches above the forms, had not been guarded to eliminate the hazard of impalement. Employees were exposed to the protruding form pins while accessing the portajohn located on the west side of the office trailer. "ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THIS HAZARD IS TO INSTALL PLASTIC REBAR CAPS OR EQUIVALENT PROTECTION ON PROTRUDING FORM PINS ".
