Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment free from recognized hazards that were likely to cause death or serious physical harm to employees working from a personnel platform elevated from a rough terrain forklift. a)DeKalb Jobsite - Employee working from an elevated platform that was not secured to the lifting carriage or forks. b)DeKalb Jobsite - Employee working form an elevated platform without fall protection. c)DeKalb Jobsite - The controls of the rough terrain forklift were left unattended while an employee worked from an elevated work platform. Among feasible means of abatement would be to comply with the American Society of Mechanical Engineers (ASME) standard B56.6a Safety Standard for Rough Terrain Forklift Trucks. NO CERTIFICATION REQUIRED FOR THIS ITEM
