hazards that were causing or likely

were exposed to    :
(a) Jobsite:  On or about May 7, 2003 an employee was climbing the midrail
and the toprail of a scissor lift to
access/egress the roof of the building.  The fall hazard to the employee
was approximately 14 1/2 feet onto
concrete.
Among other methods, one feasible and acceptable means of abatement is to
utilize fall protection.
