to crushing injuries:
(a) R Bay; The foot pedal on the Cyril Bath Co. mechanical press brake was
not guarded.
The foot pedal was approximately eight inches above the floor.
Among other methods, one feasible and acceptable method to correct this
hazard is to guard the foot pedal.

hoists in
accordance with the manufacturer's recommendations.  Daily visual checks of
these hoists are not done either.  These hoists include, but are not
limited to the
Lodestar hoists, the Budgit hoists, the Gorbel hoist and the Anchor hoist.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to develop a preventative maintenance program which incorporates al
recommendations outlined
in the manufacturer's manuals with written, signed and dated reports and
also in accordance with
ANSI B30.11 in order to ensure a safe and properly functioning hoist.oning
