Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to a crushing hazard from falling
loads:
(a) Facility, Avenue B, Leetsdale, PA:  On or about September 25, 2002, an
employee was exposed
to a crushing hazard while working under a suspended load. The employee
was working underneath
a 4242 pound bag of T60/64 TAF which was suspended from a 2 ton Shawbox
hoist.
(b) Facility, Avenue B, Leetsdale, PA:  On or about September 25, 2002,
the two ton Shawbox
box hoist was overloaded when it was being used to hoist the 4242 pound
bag of T60/64 TAF.
(c) Facility, Avenue B, Leetsdale, PA:  On or about September 25, 2002,
there were no monthly
inspections being performed on the Shawbox 2 ton hoist. In addition,
employees are not performing
daily inspections prior to use.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to develop a
preventative maintenance program which incorporates all recommendations
outlined in the manufacturer's
manuals with written, signed and dated reports and also in accordance with
ANSI B30.16 in order to ensure a
safe and proper functioning hoist.  In addition, the employer shall ensure
that no one works under the load and
that the hoist is only used for materials within it's rated capacity.

death or serious physical harm to employees in that, employees don't climb
up to the landing platform to
perform their work, exposing them to a fall hazard of approximately eight
feet:
(a) Facility, Avenue B, Leetsdale, PA:  On or about,  September 25, 2002,
employees
were exposed toa fall hazard when working from a caged ladder. Employees
untie the bottom of the "super
sack" while bracing themselves with their feet on the cage of the ladder.
The employees then put their arms/hands through the ladder's cage to
release the material contained
in the sack.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to provide a
platform with a guardrail system at the correct working height for
employees to safely unload materials into
the hopper.
